Printing Annual Report Forms

Most corporations are required to file an annual report once each year. The Next Annual Report Due date for a corporation is available on the Corporate Inquiry page on this web site. Corporations should receive their blank form with the file number, name, and address preprinted approximately two months prior to the corporation's report due date.

If you have not received your Annual Report form, or if you need a fresh copy, or if you need to file reports for previous years, you can get a copy of the form through this web site and the Corporate Inquiry screen. Follow these instructions to print a copy of the form for your corporation:

  • To begin, search for your corporation using the Business Entity Search instructions.
  • Once you have located your corporation, be sure to check the Domestic Address. This is the address to which the original form was mailed. If you did not receive your report, be sure this address is correct. Refer to the section labelled "If you move, tell us" on the Annual Reports FAQ page.
  • Check the Next Annual Report Due date. Make sure it is within the next two months. If it is more than two months away, chances are your report has not yet been mailed.
  • You can print a blank form or complete a fillable annual report online, download it and mail it in. In order to print a blank form or fill a form, click on the button marked "Forms for Annual Reports to be printed and mailed" which is in the Annual Reports section of the Corporate Inquiry page.
  • You will then be presented with one or more dates. These are the dates for which Annual Reports are outstanding for the corporation. If there are multiple dates shown, you should repeat the following steps for each date shown. However, if our records indicate that the corporation has filed all of its annual reports, you will receive a message that there are no reports due, and will not be able to continue.
  • Highlight a date by clicking once on it. Click on the Select Date button. Repeat this step for each report due.
  • Next you will be presented with some very important instructions. PLEASE READ THESE INSTRUCTIONS CAREFULLY BEFORE PROCEEDING. Scroll to the bottom of the instruction page and click on the date button. This will generate a prefilled Annual Report in Adobe PDF format. Depending on the speed of your internet connection, this might take several minutes.
  • Once the form appears on your screen, you can scroll through it with the arrow keys on your keyboard, or print it by clicking on the PRINT button at the top of the form.

Once you have printed the form(s), fill it out and return it to the Commission as you normally would. Please follow the checklist of instructions which is included as page four of the report, and is also available from this web site. In addition, you should refer to the Annual Reports FAQ page for more important information regarding Annual Reports.

* Make sure each report prints out the corporate file number, corporate name and due date. These fields must be preprinted and are not allowed to be altered.
